Those who wish to request the official COVID-19 vaccination status document must meet the following requirements:

People with a digital sign need:

1. To fill out and complete the form available on the page of Ministerio de Salud for this purpose, the form can be found here.
2. To complete the affidavit and digitally sign it, then you have to email it to [email protected]
3. A copy of the vaccination record card
4. If the vaccination record card is in a foreign language, you must present a Spanish translation
5. To have completed the Heath Pass when you entered the country after vaccination

Steps 2 to 5 must be sent together to this email: [email protected] with the first name and last name in the subject line.

People without a digital sign need:

1. To fill out and complete the form available on the page of Ministerio de Salud for this purpose, the form can be found here.
2. To complete the affidavit, sign and scan it
3. A copy of the vaccination record card
4. A copy of the ID document
5. To have completed the Heath Pass when you entered the country after vaccination
6. If the vaccination record card is in a foreign language, you must present a Spanish translation

Steps 2 to 6 must be sent together via email to: [email protected], include the first and last name in the subject line.

The process number that is generated once you have completed the online form may take time to arrive, you should not complete the form again if it does not arrive immediately, just make sure you have sent the form.
